13) Changes in Facial Expression
Hospice nurses are often attentive to the face, because it can reveal comfort or distress before words do. Near the end, the expression may soften, the jaw may relax, and the forehead may smooth out. In some cases, the person may appear distant or have a blank gaze. Nurses also watch for grimacing, tightening around the mouth, or furrowing of the brow, which may suggest pain or discomfort. Facial cues can help guide medication and positioning decisions. For families, a relaxed face can be reassuring, while tense expressions may be a clue to call the care team.
